AQUA APPLIANCE REPAIR LIABILITY WAIVER FORM

Customer Acknowledgment and Liability Release

This Liability Waiver Form is entered into by and between Aqua Appliance Repair (hereinafter referred to as "Company") and the undersigned customer (hereinafter referred to as "Customer"). By signing this form, the Customer agrees to the following terms and conditions:

  1. Authorization for Access
    The Customer grants Aqua Appliance Repair and its representatives permission to access their property and/or home to perform appliance repair, maintenance, or related services. The Customer understands and agrees that providing such access is their sole responsibility.

  2. Absence of Customer
    The Customer acknowledges and agrees that Aqua Appliance Repair may enter and perform services in the Customer's absence, with or without the Customer being present. The Customer voluntarily authorizes the Company to proceed with services under these conditions.

  3. Liability Waiver
    By signing this agreement, the Customer expressly agrees that Aqua Appliance Repair, its employees, contractors, representatives, and affiliates are NOT responsible or liable for any of the following, regardless of how it may occur:

    • Loss, theft, or misplacement of personal items.

    • Damages, scratches, dents, scuffs, stains, or any other accidental or incidental damage to property, furnishings, fixtures, appliances, or surfaces.

    • Missing or misplaced valuables, including but not limited to cash, jewelry, electronics, or personal belongings.

The Customer further understands that their decision to provide access to Aqua Appliance Repair in their absence constitutes an agreement to waive any and all claims, actions, or disputes against the Company for any aforementioned issues.

  1. Customer Responsibility
    The Customer agrees to:

    • Ensure that the work area is clear, accessible, and safe for Aqua Appliance Repair personnel to perform their duties.

    • Secure or remove any valuables, fragile items, or personal belongings from the work area to prevent potential incidents.

  2. Voluntary Agreement
    By signing this form, the Customer acknowledges that they are voluntarily waiving any right to hold Aqua Appliance Repair liable for claims, losses, or damages that may arise due to their absence during the service appointment. This waiver applies to the Customer, their representatives, heirs, and assigns.
